My wife worked at a non-profit that had a donor-tracking system will a similarly extensive list. She always set her own record in the system to simply have a title of "The".

British Airways also has a very impressive selection, including 'President', and 'Her Majesty', which are presumably used exclusively by pranksters, as well as a huge selection of foreign ('Hajah', 'Khun Ying', 'Embajadora'), and military ones (often abbreviated and non-abbreviated forms - maybe there are different conventions in different places?)

I would also add Rebbetzin as one of the titles.

I've just used the Royal Opera House ticket booking system for the first time (to see the new Tarik O'Regan/Tom Phillips opera of Heart of Darkness, since you didn't ask), and I'm afraid their 133 titles knocks yours into a three-cornered cocked hat in terms of comprehensiveness.
