Dan Schellenberg has made a nice clear screencast about how to use the new "forms" feature of Google Docs to create a form for people to fill in, with their information then fed back into a Google Spreadsheet.

What a fantastic simple data capture mechanism.
You can access a data-collection form without needing to have a Gmail or Google Docs account.
Although there is no data validation you can use radio buttons and drop downs to control input. To view a simple demonstration see input, and output .
